Financial Disclaimer

Third-Party Financing Disclaimer We have service agreements that allow us to offer third-party health financing options to help patients afford our products and services. However, all financing is originated and managed exclusively by independent third-party financing companies (e.g., CareCredit, PatientFi, Cherry, etc.).

We are not a creditor, lender, or financial institution. We do not control, underwrite, or guarantee approval decisions, interest rates, repayment terms, or any other aspect of the financing agreement. Any questions, concerns, or disputes regarding financing (including billing, collections, or account issues) must be directed to the financing provider directly.

By applying for or accepting third-party financing, you acknowledge that you are entering into a separate agreement with the third-party lender, and we bear no responsibility or liability for the terms, approval, or administration of that financing.

Third-Party Financing Disclaimer – California Notice

Financing options are offered through independent third-party lenders and are not provided by our practice. We have service agreements that allow us to make these options available, but we are not a lender, creditor, or financial institution. We do not underwrite, approve, or guarantee any financing application, interest rates, fees, repayment terms, or credit decisions. All terms and conditions are set solely by the third-party financing company.

Important California Consumer Disclosures:

  • You are applying directly for credit with the third-party lender (e.g., CareCredit, PatientFi, Cherry, etc.), not with our office. This is a separate credit agreement between you and the lender.
  • Our staff cannot fill out or submit the financing application for you. You must complete the application yourself directly with the lender.
  • You have the right to request a written treatment plan and a detailed estimate of the total cost of services/products before applying for financing.
  • You are not required to use third-party financing. Other payment options may be available — please ask our office.
  • If the financing includes a promotional or deferred-interest offer, carefully review the terms. If the balance is not paid in full within the promotional period, you may owe significant interest retroactively on the entire amount at the regular high APR.

Medical Debt Notice (Required by California Civil Code § 1785.27): NOTICE TO CALIFORNIA CONSUMER: A holder of this medical debt contract is prohibited by Section 1785.27 of the Civil Code from furnishing any information related to this debt to a consumer credit reporting agency. In addition to any other penalties allowed by law, if a person knowingly violates that section by furnishing information regarding this debt to a consumer credit reporting agency, the debt shall be void and unenforceable.

Any questions or issues regarding your financing account, billing, payments, or collections must be directed to the third-party lender directly. We are not responsible or liable for the lender’s decisions, terms, servicing, or any disputes related to the financing agreement.

By choosing to apply for or use third-party financing, you acknowledge that you are entering into a direct financial agreement with the independent lender.
